APPLICATIONS WILL BE REVIEWED ON THE FOLLOWING SCHEDULE:
Complete applications received by:
June 28th will be placed or waitlisted by August 1st
August 30th will be placed or waitlisted by September 20th
November 1st will be placed or waitlisted by December 6th
January 31st will be placed or waitlisted by February 28th
March 3rd will be placed by March 28th

How Do I Apply?
ALL APPLICANTS PLEASE COMPLETE:

REGISTRATION FORM
This form must be filled out even if you have attended sessions at our clinic before.
$50 NONREFUNDABLE PROCESSING FEE
After completing the registration form, check your email for a message from Social Thinking Stevens Creek inviting you to sign into your Client Portal
Please allow 2-3 business days for this email to arrive
You can submit payment for the processing fee through the Client Portal or you can call 408-244-2005 to pay by credit card
We cannot process your application without the processing fee
IN ADDITION TO THE ABOVE, NEW APPLICANTS* PLEASE COMPLETE:
*You are a new applicant if you have not attended a session at Social Thinking-Stevens Creek in the past 6 months
